Your Right to a Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining how much your care will cost.

Under the law, health care providers must give people who are uninsured or who are not planning to use insurance an estimate of the expected charges for services. This is called a Good Faith Estimate.

What this means for you

You have the right to receive a Good Faith Estimate for the total expected cost of any services that are not covered by insurance. This estimate covers items and services such as the intake and psychotherapy. 

You can ask Dr. Julia Zukin for a Good Faith Estimate before you schedule a service. If you schedule a service, you will receive a Good Faith Estimate in writing at least 1 business day before the service. If you request one, you will receive it within the time frame required by law.

If your bill is higher than your estimate

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill through the federal patient-provider dispute resolution process. Starting this process will not affect the quality of care you receive.

Please keep a copy or a picture of your Good Faith Estimate for your records. A Good Faith Estimate is not a contract and does not require you to obtain services from Dr. Julia Zukin.
